How do radio stations know how many people are listening? Do our radios broadcast back to the station?

I dont think our radios would have enough transmit power to reach a radio station but how else would they know how many listeners they have at one time. And to broaden things , how do television stations know whos watching? Does my cable send messages back to the cable company , then back to the television network? Does my television infringe on my privacy?

1. Surveys - for TV, they're called Nielsson's ratings, not sure how radio does it. No return messages without an extra box on the set.
